To apply for a visa, you must complete a Russian Visa Questionnaire/Application form from the Consulate. (2 copies). Use these forms, print them out, fill them in, and please make sure that:

• you have answered all the required questions on the form fully and clearly.
• you have signed and dated the form where asked.
